Tribute's PlatinumCare Flex Card

Tribute’s PlatinumCare Flex Card provides for convenient payment to be made at the time of service and at any provider who accepts debit card payments. 

Your Tribute PlatinumCare Flex Card can be used with the primary business categories of dental, vision, hearing, grocery, and pharmacy services. 

Card benefit balances can be checked by calling our Tribute Service Team and via online access to your account. Details provided upon card delivery.

Tribute Advantage members enjoy valuable additional supplemental benefits and payment for services can be made with your personal PlatinumCare Flex Card. Supplemental benefits include:

$1,500 annual for services

$325 annual for exams, lenses and frames

$1,250 annual for exams and devices

Healthy Foods PLUS

$175 per month with no rollover; to purchase groceries and needed OTC items for chronic condition plan members

Over-the-Counter Supplies (OTC)

$30 per month with no rollover
